Step
1.
Go to the
Charter Licensee application form.
Step 2.
Once we have received your application information, we will
create your Code of Ethics, with your modifications and place that into
your license agreement which will be in the form of a Microsoft Word
document. This Word document will contain the entire terms and
conditions you will agree to in their entirety. We will email this document
as an attachment to the e-mail address
given in the application form.
Step 3.
You will carefully review the license agreement, accept it by signing it,
and return it along with your annual licensing fee to Quality Testing
Labs via Federal express or any other next day service. We will provide
the address for shipping.
Step 4.
QTL will then issue you a temporary license and provide you with a URL
where you can go to get the QT Mark. Within 5 business days QTL will
contact you via email on how to link your QT Mark to your online Code of
Ethics which will reside on our QTL server. Before your online Code
becomes live, we will present it to you for final approval. Once we
receive your approval via e-mail, you then can link the Mark to that
Code.
