If it were a real Complaint Form, your Complaint would be sent directly to the Licensee as well as a QTL representative.

The Licensee would then contact you and resolve the Complaint with you to your satisfaction as well as provide some form of valuable consideration as stated in the Licensee's Code if your Complaint is valid.

Use this form to file a Complaint for Reliability Violations

Reliability Code - we agree that our Site will not be down for more than 5 minutes in any 24 hour period. This assumes that the consumer can connect to the Internet by the consumers' network. 

Violation of the Reliability Code must be verified by saving a copy of the page that was seen when you attempted to reach the Site. Save this page to your hard drive by clicking file, save, in the browser menu bar. Please remember where you saved this file.

QTL may request that you send us this page for verification.
