General Procedures for becoming a QTL Licensee:

1. Choose your Site Category 
Choose the category of service your Site provides.

2. Review your Licensee Fee
License fees are based upon the associated risk for the type of service your Site provides.

3. Submit your application
When you submit your application information and Code of Ethics, we will review it carefully. We will then contact you. Most reasonable modifications are acceptable.

4. Approve the License Agreement
Once Step 3 is complete, QTL will send you our License agreement with your Code of Ethics via e-mail to the address provided in the application form. You will carefully review the license agreement, sign it, and return it with your annual licensing fee to Quality Testing Labs via Federal express or any other next day service. We will provide the address.

5. Receive your QT Mark
Once Step 4 is complete, we will issue the Mark to you via e-mail with complete and easy to follow instructions.
