Complaint Form

The Complaint form is an online mechanism for reporting violations of posted Code of Ethics pertaining to QTL Web site licensees, or misuse of the QTL Mark. BEFORE filling out the Complaint form, we urge you to complete the following three steps.

  1. Confirm that the Web site is a QTL licensee.
  2. Verify that the complaint is a Code of Ethics matter relating specifically to the Web site.
  3. Contact the Web site licensee directly.

If the Web site does not resolve your complaint directly or satisfactorily, then you may: Submit a complaint to QTL.

Three Preliminary Steps

STEP 1. Is the Web Site a QTL Licensee?

QTL does not have jurisdiction over Web sites that do not display the QTL Mark and agree to adhere to our program's principles. As such, we can't help you resolve a privacy issue or complaint unless the Web site is a QTL licensee. 

If you feel that this Web Site is a good candidate for the QTL Mark, contact THEIR webmaster and suggest that you wish the site would become QTL certified at www.qualitytestinglabs.com.

STEP 2. Is Your Complaint a Code of Ethics Issue?

Before you file a complaint with QTL, it's important that you review the Web site's posted privacy policy carefully.  We can process and resolve issues that pertain ONLY to violation of a licensee's posted Code of Ethics or misuse of the QTL Mark. 

It is within our charter to process consumer complaints or issues regarding a Web site's specific security, privacy, commerce, online ordering, customer service and other company procedures.  

STEP 3. Have You Contacted the Web Site Directly?

After you've determined that the Web site is a QTL licensee and that your complaint is indeed a Code of Ethics issue, you should contact the Web site directly. All QTL licensees have agreed to acknowledge consumer complaints and report back to consumers with a clear account of their findings in no later than 10 days.

We ask that you contact the Web site licensee before involving QTL for two reasons:

  1. This direct approach usually resolves the issue faster.
  2. We believe our Web site licensees should have the first opportunity to address and expedite resolution of complaints.

Filing a Complaint

All complaints are sent directly to QTL AND the Web Site in question. If the Site does not contact you within the number of days as stated in the customer service section of the code of ethics, YOU MUST CONTACT QTL IMMEDIATELY. A QTL Customer Service Associate will be contacting you within 24 hours to assist you in solving your issue. Also, since the Web Site in question has failed to contact you within their own designated time frame, they will fined $100.

If the Web Site in question is ALSO found in non-compliance with the Code of Ethics, it will be fined $100 for each individual incident.

1. Submit the URL
