If it were a real Complaint Form, your Complaint would be sent directly to the Licensee as well as a QTL representative.

The Licensee would then contact you and resolve the Complaint with you to your satisfaction as well as provide some form of valuable consideration as stated in the Licensee's Code if your Complaint is valid.

Use this form to file a Complaint for Claims Code Violations

Honor all Claims we make Code - all the claims and representations we make in writing on our Site, email and correspondence are true, accurate and up to date.

Violation of the Claims Code must be verified by providing evidence of the claim made on the web Site, in writing by e-mail or postal mail. Evidence of the failure of that claim must be in writing - such as a claim for next day shipping. If the Claim is in writing on the Site, please save the page, e-mail or postal mal the Claim was made on to your hard drive by clicking on file, save in the menu of your browser, or retain a copy of the printed item.

In some circumstances we may ask you to email us or fax the pages in question and email us other information to validate the failure Claim. A QTL representative will contact you with the details.
